1. Please attach a copy of the first three pages of Passport or Birth Certificate to Registration Form. (Required only for those who have not registered with the club previously.)
2. Please ensure that you provide an Email address and be sure to check your E-mail regularly as this is our primary method of communication with parents/guardians regarding practices, fixtures etc.
3. Registration Deadline is September 12th 2009.
4. Registration Fee is $225.00 per child for the season. Cheques should be made payable to Baha Juniors.
PLEASE NOTE: If no uniform is required the fee is $175.00
5. Age requirements- 4 to 17 years.
6. Registration will take place at Winton on:
September 5th 2009 (9am-11am) and
September 12th 2009 (9am-11am) ONLY
All unpaid and late registrants will go on a waiting list.
7. All registration forms must be accompanied by a valid cheque, or cash.
8. All children registered with this club are expected to attend practice and league games.
WAIVER
